RELEASE AND INDEMNITY AGREEMENT
In consideration of Pack AMO LLC (hereafter referred to as “AMO”), agreeing to care for my pet(s), I, ______________________________________________________, agree to the following terms and conditions:
1. I understand and agree that pets are animals, which may behave unpredictably and cause property damage or bodily injury to people or other pets that are either in the care of AMO or unknown pets in the community. Pets may also escape, run away, or cause harm to themselves, including death. I am further aware that that my pet(s) will be in very close proximity to other pets which are under the care of AMO and that there is a danger of disease, parasites, fights, and injury, which could lead to the death of my pet(s) and other pets in the care of AMO. I acknowledge and affirm that my pet(s) are up-to-date with all required vaccines (including kennel cough), and have received up to date flea and tick preventative medication. I will not hold AMO responsible for any action related to the above.
2. I understand and agree to allow AMO, and its staff, agents, employees, representatives, and contractors to provide care, training, and related services for my pet(s) at AMO’s address in Iowa City, IA. and at any other location they may travel to on foot or by automobile. I understand that other pets and people, both affiliated and unaffiliated with AMO, may be in very close proximity with my pet(s) while my pet(s) are in the care of AMO.
3. I hereby allow and hold AMO and its agents, employees, insurance carriers, representatives, independent contractors, successors, and assigns (all hereafter referred to as the "Released Parties"), accountable of all liability related to my dog being a doggie mentor, helping work with problematic dogs, including all claims, demands, and causes of action of every nature affecting me or my pet(s), which I may have or ever claim to have for only injury my pet(s) or for injury or death of another pet or other person or for property damage, caused by the actions or condition of my pet(s) while my pet(s) are in the care of AMO, its agents, employees, representatives, or contractors. AMO Pet Care will accept full financial responsibility for my pet(s) and any damage and/or injury that my pet(s) has caused by action or presence.
4. That this release covers all injuries and damages, whether known or not, and which may hereafter appear or develop arising from the matters above referred to.
5. That I am executing this Release solely in reliance upon my own knowledge, belief, and judgments made by the Released Parties or others on their behalf. I have reviewed this Release and understand its effect. I am not executing this Release under duress or coercion.
6. I am aware that my dog will be inclose proximity to other behavioral challenged dogs.
